Athlete Agent - Credentialing

1). Submit the completed application including all required documentation and required fees.

2). If you hold an athlete agent certificate of registration or license from another state, you may submit a copy of the application and the certificate of registration or license issued by the other state in lieu of the application, provided the application in the other state was submitted no more than 6 months prior to information required by this application and the aplication was signed by you under the penalty of perjury.

3). Department review of application; determination to grant credential or notice of denial.

Biennial renewal due by July 1 of the even-numbered years

Common Denial Reason:  incomplete application; conviction of a crime; engaged in unprofessional conduct; fraudulent representation
