Applications must meet all requirements contained in Wis Admin Code RL 61, and Section 440.62(2) of the Wisconsin statutes. These requirements include, but are not limited to, providing information on ownership, finances, floor plan, and instructors, as well as providing verification of appropriate bonding to the Department.

Biennial renewal due by March 31 of odd-numbered years
Common Denial Reason: Incomplete application; bonding insufficient; contract with students or catalog for students does not satisfy requirements.
Convictions of crimes or pending charges may be grounds for denial of license if the circumstances of the conviction or charge are substantially related to professional practice.
