Applicants for licensure by endorsement/reciprocity must hold a current license to practice dental hygiene in another state which has not been suspended or revoked, where the requirements imposed are substantially equivalent to those of this state. The applicant must have engaged in at least 350 hours of active practice of dental hygiene in the 12 month period preceding application for licensure in Wisconsin. Applicants will be required to submit original national examination scores, original clinical and laboratory examination scores and pass the Wisconsin open-book statutes and rules examination. Applicants will also be required to submit educational information, professional recommendations and proof of possessing a current certificate of proficiency in cardiopulmonary resuscitation.

How long does it take to make a determination to provide credentials?

By law an original completed application for a credential shall be reviewed within 60 business days after having been received, but the average time for review is 30 days. An application is considered complete when all materials necessary to make a determination on the application and all requested materials have been received.
