Can
credentials be transferred from other States/Countries?
Complete
the appropriate application. Applicants who have ever been credentialed
in another state will be required to have each state board submit
a verification of licensure to the Wisconsin Dentistry Examining
Board.
Applicants for licensure by endorsement/reciprocity
must hold a current license which has not been suspended or revoked,
in a state where the requirements imposed are substantially equivalent
to those of Wisconsin. The applicant must have actively engaged
in the practice of dentistry for 750 hours within the 12 month
period preceding application, and must also have 750 hours of
active practice during three other 12 month periods in the 5 years
before application as a licensed dentist in the state of Wisconsin.
Applicants will be required to submit original national examination
scores, original clinical and laboratory examination scores and
pass the Wisconsin open-book statutes and rules examination. Applicants
will also be required to submit educational information, professional
recommendations and proof of possessing a current certificate
of proficiency in cardiopulmonary resuscitation.
