SPECIAL NOTE TO PHYSICIANS
AND OTHER PRACTITIONERS CONCERNING NATIONAL PROVIDER IDENTIFIERS
(NPIs)
If you voluntarily surrender your license or retire and you
will no longer be furnishing health care in this or any other
State, please remember to deactivate your NPI in the National
Plan and Provider Enumeration System (NPPES). Deactivating your
NPI will help ensure that your NPI is not used fraudulently
by others. If you do not know how to deactivate your NPI, you
may contact the NPI Enumerator at 1-800-465-3203.

Wisconsin Act 104, relating to requirements to successfully
complete training on use of an automated external defibrillator.
Rules and
permit requirements regarding the adminsitration of anesthesia
or sedation
On May 3, 2006,
the Dentistry Examining Board adopted rule changes to Wis.
Admin. Code s. DE 11. The rule required dentists administering
anesthesia or sedation (other than nitrous oxide inhalation
or anxiolysis) to obtain a permit from the Board by January
1, 2007.
Three classes of
permits will be issued, at these levels:
1) Class I - Enteral conscious sedation;
2) Class II - Parenteral conscious sedation;
3) Class III - General anesthesia or deep sedation.
To qualify for a permit, the didactic and clinical training
requirements specified in the rule must be met. For further

APPROVED LIST PROCESSING YOUR APPLICATION WILL REQUIRE ADDITIONAL
TIME. Application for Dental Permit
to Administer Conscious Sedation
If providers have
not been board approved, they may apply for approval by submitting
a syllabus that specifies the content, hours and clinical
cases contained in the training to Office of Education and
Examinations, Department of Regulation and Licensing, PO Box
8935, Madison, WI 53708. The materials will be referred to
the Dentistry Examining Board for review.
New Examination
Requirements:
At its meeting on
July 1, 2009, the Board decided to accept the following
examinations for both dentists and dental hygienists: CRDTS,
WREB, NERB, SRTA, ADEX, and CITA. The ruling is in effect
for examinations taken on or after JANUARY 1, 2009.
Note: Parts I and
II of ADEX are not required for WREB examinees for examinations
taken on or after JANUARY 1, 2009.
At its meeting
on September 28, 2005 , the Dentistry Board decided to accept
the Northeast Regional Board (NERB) examination for
Wisconsin licensure in both Dentistry and Dental Hygiene.
Acceptance is retroactive for 5 years from September
28, 2005.
"Effective December
1, 2005, applicants completing the Western Regional Examining
Board examination must also successfully complete the ADEX
exam portions Part I in diagnostic and treatment planning
and Part III of the mannequin examination in fixed prosthodontics.
Applicants who have successfully passed the WREB examination
prior to December 1, 2005 are not required to pass ADEX
I and III"
