1). Complete high school
education or equivalency.
2). Complete and
submit the application form,
including all required documentation.
Please note: the
application must be completed 30 days prior to the examination date.
3). An applicant
for permanent licensure may apply for a temporary permit at the same
time but must pay both fees and meet the following conditions:
a) Fulfill the requirements regarding
age, character and high school education.
b) It
is highly recommended that all candidates contact the Department
for a trainee permit. A temporary permit allows candidates to practice
fitting hearing instruments, make ear impressions and measurements
of human hearing for the purpose of fitting hearing instruments
for a period of one year while under the direct supervision of a
licensed hearing instrument specialist. Candidates who have not
had a temporary permit and experience will be at a significant disadvantage
in being prepared to take the practical and written examinations
for licensure. A person holding a valid hearing instrument specialist
license shall be responsible for the direct supervision and training
of the applicant and shall be liable for all negligent acts and
omissions of the trainee in the fitting of hearing aids. Please
note that the name of the licensee who will be supervising the trainee
must appear on the face of the temporary permit.
Please
note: if a temporary permit is desired, an applicant must apply
for both the temporary permit and the permanent license at the same
time.
4). Pass
required examinations.
5). Board
review of application, determination to grant credential.

Biennial
Renewal Due: by January 31 of odd-numbered years
Common Denial Reason:
Ineligibility, conviction of substantially related crime.
Convictions
of crimes or pending charges may be grounds for denial of license
if the circumstances of the conviction or charge are substantially
related to professional practice.
