Instructions for Ordering Computer Reports:

The order form is now available to print. If you want the form mailed to you, call (608) 267-1812 and leave a message including a mailing address, fax number, or email address and we will send you our List Order Form Packet.  Please include your phone number in the event we need to contact you.

A computer printout contains names, addresses, license numbers, dates granted and latest renewal dates for all credential types the department handles. Lists may be ordered in zip code or alphabetical order. An order may include credential holders with Wisconsin addresses only OR credential holders residing in-state and out-of-state.

Note: Individuals may request that their name, street address, post-office box number or 9-digit extended zip code be withheld from lists of 10 or more credential holders (440.14, Wis. Stats). This “opt-out” choice is not available to a business entity unless it is a sole proprietorship or for lists that the Department furnishes to another state agency, a law enforcement agency, a federal governmental agency or the coordinated licensure information system under s. 441.50(7).

CD:

Lists can be provided on a CD in comma delimited format.  Information provided on the CD is the same information as on a "list" but with fixed length fields.  The data will be in comma delimited format (xxx,nnn).  A README.TXT file is included on the CD with a record layout.

EMAIL:

Lists can be provided by email.   Information provided by email is the same information as on a “list” but with fixed length fields.  The data will be in comma delimited format (xxx,nnn).   A README.TXT file is included in the email with a record layout.  Other applicable attachments will also be included.   If the file is too large, it will be put on a CD.
