Pharmacist - FAQ

How can I find out if they have credentials?

Visit the License Lookup , click on and select from the Credential Type, type in the last name or credential number to search.

How can I file a complaint?

Go to File a Complaint and follow the steps

How can I find out if there is discipline or other orders affecting a credential holder?

Visit the Reports of Decisions and type in their name in the Search window.

Can credentials be transferred from other States/Countries?

A person who is currently registered to practice as a pharmacist in another state that reciprocates with Wisconsin and who has graduated from a board approved school, college or department of pharmacy, may file an application for licensure by reciprocity. Please contact the Department for the appropriate form.

Stages in Determination Process:

1. Complete the preliminary application form and forward to NABP.

2. Complete Eligibility for Transfer of Pharmaceutical Licensure Based on Active Practice of Pharmacy form to determine whether any state board exams can be waived.

3. Complete the state application form that you receive from the NABP.

4. Return completed application form with fee to the Department. Application, fee, and all credentials must be on file not later than 45 days prior to the date of the examination.

5. Members of the board will review your completed active practice of pharmacy form to determine what examinations will be required. An applicant who is not engaged in the active practice of pharmacy shall take the MPJE examination and the patient consultation examination. The patient consultation examination is offered during the months of January, March, June, July, August and October.  An applicant who is engaged in the active practice of pharmacy shall take the MPJE examination only.

People Trained Outside the United States:

Graduates of schools of pharmacy whose professional degree programs are not accredited by the American Council of Pharmacy Education (ACPE) may pursue licensure as a pharmacist in Wisconsin by a pathway which includes certification by the Foreign Pharmacy Graduate Examination Committee (FPGEC) and successful completion of 1500 hours of internship.

How long does it take to make a determination to provide credentials?

By law an original completed application for a credential shall be reviewed within 60 business days after having been received, but the average time for review is 30 days. An application is considered completed when all materials necessary to make a determination on the application and all requested materials have been received.
