Registered Nurse by Examination - Application Forms

Petitions for refunds must be in writing and may be granted only if the criteria set forth in Wis. Admin. Code s. RL 4.06 are met.

Statement of Graduation Exam (For board approved schools only, US and US Territory.)
Statement of Foreign Nursing Education (For foreign graduates only, this includes Canada.)
Request For Temporary Permit For Registered Nurse or Licensed Practical Nurse (Can only be processed if complete application form #739 is received with all required fees.)
Verification of Licensure (For foreign graduates only, this includes Canada.)
You must register for this exam. It is important to take this test, and if needed , to retest promptly. Per the Board of Nursing decision in April 2004, a nursing graduate who does not hold a temporary permit or who has failed this exam may continue to work as a nurse tech for up to 6 months past his/her date of graduation.
